Pay: $21.67–$26.26/HR (Based on Experience)
Schedule: ONC/PT | Weekend Availability Required

What You’ll Do
Help residents adjust to their new environment with compassion and care
Develop social histories, psychosocial assessments, and discharge plans
Support residents and families during transitions and emotional challenges
Document progress notes and contribute to care plans
Foster a positive environment through volunteer programs and family group meetings
Uphold residents’ rights and ensure personalized attention to individual needs
What We’re Looking For
Preferred: Bachelor’s degree in Social Work or related field
Healthcare experience highly preferred
Ability to connect positively with residents, families, and team members
Must meet applicable State and Federal requirements
Comfortable using basic computer systems and documentation tools
